SUBMITTING YOUR TESTING INFORMATION

Important: Please read all instructions carefully before mailing your AccuStar long-term test kit. Make sure to complete the form.

Completing the Laboratory Form

This form is essential. It is the only way the laboratory can associate your dosimeter with you (by name, email, and address).


Without it, the lab will have no way of knowing who the dosimeter belongs to, and it cannot be processed. Complete the form here.


Setting Your Test Stop Reminder

When you fill out the form, you’ll be asked for a date to receive a reminder to end your test. Please keep in mind that these reminder emails can sometimes end up in spam folders. We strongly recommend adding a calendar reminder as a precaution.

In the bottom right of the page, enter your 7-digit device number and the postal code of the test location.

You’ll be taken to the test info page where you can enter your test end date, answer the security question, and submit.

Before mailing your test

Carefully review the checklist in your user manual to ensure all steps are complete.


When mailing, we strongly recommend using tracked shipping, as the lab does not provide receipt confirmation.


It is your responsibility to keep record of your device number, as we do not keep a record of it.


Important


If the form isn’t filled out and you don’t have your device number, there will be no way for the lab to locate or process your detector.
